Initial evaluation sessions are scheduled for 90 minutes. We prefer these sessions to be at our office, but can be via TeleHealth if needed.
You will need to bring your photo ID and insurance card or Medicare/Medicaid card. Bring a list of your current medications, doses, and dosing instructions. This may be obtained at your local pharmacy.
Please complete our pre-evaluation forms before your initial evaluation. You may download them here and bring the completed forms to your appointment. Alternatively, you may arrive 15 minutes early to complete these forms in-person.
Follow-up medication management appointments are scheduled for 30 minutes. These sessions may be in-person at our office or via Telehealth.
We suggest scheduling appointments 4-6 weeks in advance.
Please give at least 24 hours notice if you are unable to attend an appointment. If notice is not given with more than 24 hours notice, you will be charged a late cancelation fee.
Appointments will be cancelled and must be rescheduled if you are more than 10 minutes late. If you do not arrive for an appointment, you will be charged a no-show fee.
Contact your pharmacy FIRST and request that the Pharmacy contacts our office to request more refills. Generally, this process will result in faster refills. This may not apply to certain medications.
Medication refill requests may be denied if you do not attend your medication management appointments on a regular basis. It is important that we monitor your medications on a regular basis!

Payments may be made by cash, check, or credit/debit card in person. Checks may be mailed to their PO Box listed above. Credit/debit card payments may be made over the phone.
